clamps included with this, or do I need to order them separate?

Stainless steel clamps come with it. That said, the installation specs call for a minimum of two clamps at each end, and the recommended styles are 3/4 inch wide T-bolt clamps or 5/8 inch wide constant torque clamps. If you need extras beyond what's included, you'd want to pick those up separately.

got a 5 inch wet exhaust run and need to kick it over 45 degrees. is this elbow actually 5.00" ID, and how much straight length do i get for the clamps?

It is a 5.00 inch inside diameter elbow with a minimum clampable length of 1.62 inches. Plan on using at least two stainless steel clamps on each end, and the recommended styles are T-bolt or constant torque clamps.
