Does this seal work on both the 3000 and 5000 series Permco units, or is it just one of them?

Covers both. The part is built specifically for Permco 3000 and 5000 series equipment, so you don't need separate seals if you're running both in the same shop. Supplier part number is W62-26-16, and it also crosses to alternate part number C1DT23 if that helps with your records.
