What's the stroke on this cylinder, fully extended? Just want to make sure it'll fit my setup.

Pin to pin, you're looking at 42.25 inches when it's fully extended. Retracted, it's 26.25 inches.

rated load says 12,800 lbs but the pressure rating shows 1800 PSI, which one am i actually limited by in normal operation?

At standard working pressure of 1800 PSI, the 12,800 lb rated load is what you can expect from the push stroke on that 3-inch bore. The 2500 PSI figure is the maximum operating ceiling, not your everyday working number. So under normal conditions, 1800 PSI and 12,800 lbs are the figures to plan around. Keep in mind that on full extension with a 16-inch stroke, column loading becomes a real factor too. Long strokes reduce the safe push load because of buckling risk on the rod, so the rated load assumes a minimum safety factor of 2 and is calculated on a pin-to-pin mounting basis.

got a 1/2 npt hose setup already. are the ports on this cylinder 1/2 npt or am i gonna need adapters

Port size matches what you are running. It comes with 1/2 NPT ports.
