trying to replace a tee on a skid, is this the one with 1/2-14 male pipe on the run and 3/4-16 male JIC on the other two ports?

It matches that setup. The run side is 1/2-14 male NPTF, and both the run and branch on the flare side are 3/4-16 male SAE 37 degree (JIC) connections.
