So this is an elbow, right? Does it come with fittings, or do I need to get those separate?

This is just the adapter itself. You'll need to source your own fittings to complete the connection.

got a tight spot on a line, is this the 45 degree elbow that goes from 1-11-1/2 female NPT to 1-11-1/2 male NPT?

For a simple direction change without changing thread size, it fits that job. It is a steel 45 degree elbow adapter with 1-11-1/2 female NPTF on one end and 1-11-1/2 male NPTF on the other, in the Eaton Aeroquip 2088 series.
