trying to replace an old flange to pipe adapter, is this one the 1-1/2 code 62 4-bolt flange to 1-1/2 female npt straight style?

Match-up wise, it’s a straight steel adapter that goes from a 1.50 inch Code 62 four-bolt flange face on the O-ring half to a 1.50 inch female NPT port. The pad size is 1.5 and it’s listed as fitting 1.5 inch pipe size, so it’s the common 1-1/2 to 1-1/2 flange-to-NPT transition.

Does this adapter come with everything needed to bolt it up, or do I have to track down the hardware separate?
