Does this thing come with everything I need to bolt it up, or do I gotta track down the hardware separate?

It ships with the mounting bolts, lock washers, and a Buna-N O-ring all included. Nothing extra to source before you can make the connection. The flange side is SAE Code 61 four-bolt, and the other end is a 1-1/2 inch female NPT thread, so as long as your port and pipe sizes match up, you're ready to go straight out of the box.

got a 1-1/2 Code 61 4-bolt port and need to turn 90 degrees into female NPT. is this the right adapter?

For that hookup, it matches up clean. One end is a 1.50 inch SAE Code 61 four-bolt flange pad with an O-ring seal, and the other end is a 1-1/2-11-1/2 female NPT thread, built as a 90 degree elbow.
