got a hose that measures right at 4 inches across. is this clamp gonna be too tight or does it cover that size

It is made for inside diameters from 4.00 inches up to 4.25 inches, so a 4.00 inch diameter is within the stated range.

clamp says 4.00 to 4.25 inch ID -- does that mean it'll work on a hose that measures right at 4 inches or does it need to be a little bigger to get a good bite?

Right at 4.00 inches puts you at the very bottom of the range, so it will fit. T-bolt clamps work by compressing around the outside of the hose or tube, and this one is built to handle anything from 4.00 up to 4.25 inches. Just make sure your measurement is the outer diameter of whatever you're clamping, not the bore of the hose itself. As long as you're in that window, you'll get a solid, secure connection.
