What's the wrench size needed to tighten this fitting down properly?

The flats on this fitting measure 17.5mm across, so you'll need a wrench sized accordingly.

got a port that’s R 3/8-19 on one side and my hose end is 3/8-18 nptf. is this the right little adapter to tie em together

For a straight thread changeover between those two standards, it matches what you described. One end is 3/8 inch Male BSPT with the R 3/8-19 callout, and the other end is 3/8 inch Male NPTF with the 3/8-18 callout.

Got a piece of equipment with a BSPT port on it, trying to tie it into our NPT hydraulic connections. This the right kind of fitting for that?

That's what this fitting handles. One end is 3/8" Male BSPT (R 3/8-19) and the other is 3/8" Male NPTF (3/8-18), so it bridges those two thread standards in a single straight piece.
