fitting's got a DIN tube side marked M20 x 1.5 with an S12 size -- does that connect straight over to a 3/8 male NPT port without any adapters in between?

Correct on both ends. Side one is the DIN tube connection at M20 x 1.5 with an S12 tube size, and side two is 3/8 inch male NPT with a 3/8-18 NPTF callout. The shape is straight, so there's no offset or angle involved. As long as your port is a standard 3/8 female NPT, this fitting connects the two directly with nothing else needed in between.
