Both sides on this thing are M18 x 1.5 male thread, does that mean it's fully symmetrical, or is there some difference between the two ends I ought to know about?

Symmetrical is exactly right. Both sides carry the same M18 x 1.5 male thread in an L12 DIN tube size, so there's no designated inlet or outlet end. Either side connects the same way, which makes installation a little more straightforward when you're working through a panel and don't have a lot of room to maneuver.

got a stainless bulkhead fitting that takes DIN tube on both sides, is this one the straight-through style or is it stepped somehow?

Straight-through is what you are getting here. It is a stainless steel bulkhead union with DIN tube connections on both sides, and both ends are the same size and thread, M18 x 1.5 male for L12 tube.
