Is this a direct-replacement pump or am I gonna need to modify something to get it to fit right?

Permco built the P124A series as a direct-replacement unit, so the intent is bolt-on fitment without modification. That said, displacement, port configuration, and mounting details should be confirmed against the full P124A series spec sheet before you pull the old pump and start wrenching. Part numbers in this series encode a lot of that configuration info, so cross-referencing the number you're replacing against P124A085MLZA20-54 is the right move before ordering.
