fitting says 12 x 10mm -- does that mean the banjo bolt hole is 10mm and the outer ring part is 12mm, or is it the other way around?

The 12mm refers to the diameter of the banjo fitting itself, and the 10mm is the size of the banjo bolt that passes through it. So when you're matching this up to your brake line or caliper port, you're looking for a 10mm bolt thread with a 12mm banjo body.
