what's the deal with the "extended" part of this fitting, is that just about the length or does it change how it threads in?

Extended refers to the physical length of the fitting body, not the thread spec itself. The M10 x 1 threading stays consistent, but the longer design gives you more reach during installation, which helps a lot when you're working in tight or hard-to-access areas where a standard-length fitting would be awkward to seat properly. Brakequip built that extra length in specifically to make complex hydraulic setups easier to work with.
