what size tubing does this run tee work with, and are all three ports the same thread?

All three ports carry 9/16-18 threads, so the threading is consistent across the tee. The tube outer diameter on this forged piece is 1/4 inch, which tells you what size tubing it's built to connect with. The two male ports share the same drill size of 0.172, and the female swivel port runs a 0.165 drill, so there is a slight difference there worth knowing before you put it in line.

trying to replace a tee on a 1/4 tube line, are all three ends on this one 9/16-18?

All three ports on the 04MFS-04FFSS-04MFS run tee are 9/16-18 thread. It is also built around a 1/4 inch tube OD, so it matches that tube size in the Brennan FS6602-FG series.
