got a swivel that needs to go from 1-1/16-12 JIC flare on one side to 1-1/16-12 ORB on the other. is that what this FS65009-1212-01 is?

Match-up wise, FS65009-1212-01 is a male 1-1/16-12 JIC 37 degree flare on the casing end to a male 1-1/16-12 SAE O-Ring Boss straight thread on the sleeve end, both in a 3/4 nominal size.

What kind of seal does this swivel joint come with, and are there other options if I'm running something other than standard hydraulic oil?

Buna-N is the standard seal, which works fine for most petroleum-based hydraulic fluids. If your system runs something more aggressive, there are two other options available. The -04 suffix gets you EPR seals, and the -06 suffix is Viton. So depending on what fluid you're pushing through it, you've got some flexibility in how you spec it out.
