Got a port on my manifold that's at an awkward angle and a straight fitting just ain't gonna work. This 45-degree elbow connect a flat-face seal male end to an ORB port, right?

Correct setup. One end is a 1-1/4 in. male flat-face seal thread and the other is a 1-1/4 in. male O-ring boss. So if your manifold has a 1-1/4 ORB port and you're running a flat-face seal line, this fitting bridges those two thread standards at a 45-degree angle. That angle is specifically useful when straight or 90-degree fittings don't give you the routing clearance you need.

trying to swap out a straight fitting, what exactly are the ends on this 45 elbow?

On one side you have a 1-1/4 inch MFS male flat-face seal end, and on the other side you have a 1-1/4 inch male O-ring boss (ORB) end meant to thread into an ORB port. It is used to transition between those two connection styles while changing direction 45 degrees.
