Trying to match up a hose end, is this one meant for a 1/2 inch hose ID and a 1/2 male NPTF on the other side?

fitting says "permanent" in the short description, does that mean it can't be reused once it's crimped onto the hose?

Permanent fittings are meant to stay on once assembled, yeah. The designation refers to how the fitting attaches to the hose rather than being a field-reusable skive or push-on style. Once it's crimped, that connection is fixed. It's standard practice on high-demand hose assemblies where you want the joint to stay put and not be disturbed between service intervals.
