mounting bolts included with this valve or do I need to order them separate?

The bolts are not included. You'll need four 1/4-20 x 1-1/2 Grade 8 socket head cap screws, and Eaton sells those as Bolt Kit 255633 if you want the matched set. Torque them to 112 lb.in. max with lubricated threads. Worth doing it right the first time on a 3000 PSI application.
