Got a line that needs to make a 90-degree turn comin' off an NPT port, threads are 3/8-18. Other end has to hook up to a 3/4-16 SAE 45 flare. This the right piece for that?

That's exactly what this fitting does. One end is 3/8-18 Male NPT, the other is 3/4-16 Male SAE 45 degree flare, and the 90-degree elbow geometry handles the direction change between them. Overall it measures 1.12 by 1.22 inches, so it stays pretty compact even with the bend built in. Brass construction holds up well across a range of fluid media too.

got a tight spot by the pump. is this the 90 degree elbow that goes from 3/8-18 male npt over to 3/4-16 male sae 45 flare?

For a quick fit check, the ends on this elbow are 3/8-18 Male NPT on one side and 3/4-16 Male SAE 45 degree flare on the other, with a 90 degree bend to turn the line in a confined area.
