Trying to tuck a line in tight, is this one of those 90 degree elbows with male NPT on both ends?

For tight routing and cleaner layouts, it gives you a 90 degree turn with 1/8-27 male NPT threads on both sides, so it works as a pipe-to-pipe elbow adapter.

does this thing need any thread tape or sealant when you put it in, or is it good to go as-is?

No sealant needed. Midland Industries machines these to tight enough tolerances that the threads seal on their own. Just make sure your mating threads are clean and in good condition before you run it in.
