got a gauge port on a old grease manifold thats 1/4 NPT female and the fitting i need to hook to it is 1/8 NPT male, will this adapter get me there or do i need something different

Female on both ends is the key detail here. This adapter has a 1/4-18 female port on one side and a 1/8-27 female port on the other, so it's built to connect two male-threaded components together, not a female port directly to a male fitting. If your manifold port is female and your incoming fitting is male, you'd be threading that male fitting into one end of this adapter, which works fine as long as the thread sizes line up with those two sizes.
