Does this adapter go from a JIC fitting on one end to a pipe thread on the other, or did I read that backwards?

You got it right. The -04 end has a 37-degree male JIC flare, and the other end is a 1/4 inch male NPT. So you're threading the NPT side into a port or female pipe fitting, and the JIC side connects to your hose or line with a 37-degree flare seat.
