safety clip on this thing, does it actually keep it from poppin loose on its own, or is that more of a "just in case" feature?

Those cast-in lugs and the special cam lever arms work together so a safety pin can be inserted once the coupler is locked. Until that pin gets pulled out, the coupler physically cannot be uncoupled. It's not just a backup measure, it's a positive lock that prevents accidental release during operation.

got a 4 inch camlock setup already. is this the female half with 4" male npt on the back side?

You are looking at a female cam and groove coupler with a 4 inch male NPT threaded end. It is made to interchange with other cam and groove parts built to the A-A-59326 standard.

Will this cam lock hook up to standard fittings, or is there some special kind I gotta get?

Interchangeability is a key feature. It'll work with any product made to the A-A-59326 standard, which used to be MIL-C-27487F. Just a heads up, though, the 1/2" and 8" sizes sometimes don't play well with other brands.
