trying to connect a JIC line to a metric port on a piece of equipment and need a 90 degree turn in there, does this adapter actually lock in place once you get the angle set, or does it stay loose?

Once you get the elbow rotated to the angle you need, you tighten it down and it stays put. That adjustable feature is there specifically for situations where routing a straight adapter just won't work. You set the orientation before final assembly, lock it, and it holds. Makes a real difference in tight spots where the hose or line has to come off at a specific angle.

got a hose end that’s 5/8 JIC. will this elbow screw right onto that and then into an M22 x 1.5 port?

For connecting those two standards, it matches up as a 5/8 inch male JIC with 7/8-14 UNF threads on one side, and a male metric M22 x 1.5 on the other. It is meant to join a JIC side to a metric port at a 90 degree angle.
