got a hose end that’s 3/4-16 UNF JIC male, will this screw right into it and give me an M12 x 1.5 male on the other side?

For tying two standards together, that’s exactly what it’s built for: one end is a male JIC with 3/4 - 16 UNF threads in a 1/2 inch size, and the other end is a male Metric M12 x 1.5 in a 12 mm size with a seal.

Got a metric port on a hydraulic block and need to tie into a JIC line, this a straight shot or is there any offset to it?

Straight through, no offset. The fitting runs male JIC on one end and male Metric on the other in a direct inline configuration, so it won't add any angular deviation to your hydraulic connections. The JIC side is 3/4-16 UNF at a 1/2" size, and the Metric end is M12 x 1.5 at 12mm. Overall length comes in at 36.9mm, so factor that into your clearance before you go wrenching it in.
