Got a piece of equipment here with an M12 x 1.5 metric port and I need to tie into a JIC line, this the right adapter for that?

That's what this one's built for. One end is M12 x 1.5 metric male, and the other is a 9/16-18 UNF JIC male at 3/8 inch. Both ends are male, so you'd be threading this into female-ported fittings or hose ends on each side. The seal is included on the metric end, so you're not chasing down a separate crush washer or O-ring to make up that connection.

For that mix of threads, it matches what you described. One side is a male JIC 9/16-18 UNF (3/8 inch) and the other side is a male metric M12 x 1.5 (12 mm), and it includes a seal on the metric end.
