trying to hook a 3/4-16 JIC line up to an M18x1.5 port, is this the right elbow for that?

For that mix of threads, yes. Side 1 is a male JIC with 3/4 - 16 UNF threads, and Side 2 is a male metric with M18 x 1.5 threads. It is a 45° elbow adapter meant to bridge JIC to metric in a hydraulic connection.
