both sides are the same size on this thing, right? 5/8 BSPP male going into a 5/8 BSPP female?

Correct on both counts. The male side is G 5/8-14 and the female side is also G 5/8-14, so you're working with a same-size configuration all the way through. The elbow just changes the direction of flow at 90 degrees without stepping up or down in thread size.

trying to clean up a tight hose run, is this a 90-degree elbow with 5/8 BSPP on both ends?

For a right-angle turn, it gives you a 90° elbow with a male G 5/8 - 14 (5/8 inch BSPP) on one side and a female G 5/8 - 14 (5/8 inch BSPP) on the other.
