trying to swap a fitting out, are the ends on this elbow 1/4 JIC male and 3/8 BSPP male?

On the JIC side it is a 7/16-20 UNF male (1/4 inch JIC), and on the other side it is a G 3/8-19 male BSPP (3/8 inch BSPP).

got a machine running JIC on one end and BSPP on the other, does this fitting actually handle both thread types in one piece?

That's what it's built for. One side is 7/16-20 UNF male JIC, which is your 1/4" JIC connection, and the other side is G 3/8-19 male BSPP. Both threads are on the same elbow, so you're not stacking adapters to bridge those two standards. Steel construction keeps it solid in working conditions where a softer material would give you trouble over time.
