What's the thread size on both ends of this fitting, and are they the same type or different?

Both ends are 1/8" in size, but they're not the same thread type. One end is BSPP, which is a parallel thread with a G 1/8 - 28 callout, and the other is BSPT, a tapered thread with an R 1/8 - 28 callout. That combination is exactly what makes this fitting useful when you're connecting between systems or components that use those two different British Standard thread types.

What's the wrench size needed for tightening this elbow?

This fitting has a 12mm wrench flat, so that's the size you'll need.
