got a fitting with R 3/8 - 19 on one side and 3/8 - 18 NPTF on the other, is that what this adapter is for?

For tying those two thread standards together, that is exactly the job of this adapter. One end is an R 3/8 - 19 BSPT male, and the other end is a 3/8 - 18 NPTF female, so it lets you transition between BSPT and NPTF in a straight steel fitting.
