I need a swivel fitting to connect a 3/8" BSPP male to a 1/2" BSPP female. Will this work?

Yes sir, this straight swivel is exactly that. It's got a 3/8" BSPP male on one side and a 1/2" BSPP female on the other.

what's the thread size difference between the two ends on this fitting, is one end bigger than the other?

Yeah, the two sides are different sizes. Side one is a 3/8" male BSPP thread with a G 3/8-19 callout, and side two is a 1/2" female BSPP thread with a G 1/2-14 callout. So you're stepping up from a 3/8 male connection to a 1/2 female connection. The overall length runs 41mm, and the wrench widths are 22mm on the smaller end and 27mm on the larger end.
