trying to replace a bent elbow on a line, is this the one that goes from M20 x 1.5 DIN tube (S12) over to an M18 x 1.5 male metric thread?

It matches an M20 x 1.5 DIN tube end on Side 1 (S12) and an M18 x 1.5 male metric thread on Side 2 (18mm). It is a 45 degree elbow, so it’s meant for routing where a straight fitting won’t work.

This fitting is designed to connect an M20 x 1.5 DIN tube end to an M18 x 1.5 male metric thread. It's made for hydraulic line routing where a straight connection won't quite do.

fitting's got a 45 degree angle on it, does that adjustable end let you spin it into position after you get it threaded up, or do you have to have everything lined out before you tighten it down?

The adjustable metric side is built to let you dial in the orientation before you lock it off, so you don't have to have your lines perfectly staged ahead of time. That's the whole point of the adjustable end. Get it threaded, swing it to where it needs to sit, then tighten it down. No need to modify the line itself to get the angle right.
