What exactly are the two thread sizes on this fitting, and are they both male?

Both sides are male, yes. Side 1 is a DIN Tube connection with an M18 x 1.5 thread and an S10 size designation. Side 2 is a Metric connection, M16 x 1.5, with a 16mm size. So if you're routing from a DIN tube port over to a metric male connection, this elbow handles that transition at a 45 degree angle.

trying to swap an elbow on a line, is this the 45 degree one with m18 x 1.5 on one end and m16 x 1.5 on the other?

For a 45° elbow setup, it matches those thread callouts. Side 1 is a DIN tube connection with an M18 x 1.5 callout in size S10, and Side 2 is a male metric end with an M16 x 1.5 callout in 16 mm size.
