does the o-ring come with it or do i need to source that separate?

NBR o-ring material is included as part of the seal on this fitting. The adjustable O-ring boss end (the 3/4-16 SAE side) uses that NBR seal, so you won't need to track one down separately to make up the connection.

What size threads does this elbow have?

It comes with a 3/4-16 male SAE adjustable O-ring boss thread and a 1/2-14 male NPTF thread.
