both threads on this elbow are 9/16-18 JIC, so is this a same-size fitting on both ends or am I missing something?

You're reading it right. Both the male JIC end and the female JIC swivel end are 9/16-18, which means this is a straight-size 90-degree elbow with no reduction. That setup is pretty common when you just need to change direction in a hydraulic line without stepping up or down in thread size.
