What size threads does this elbow have?

It uses 1/4-18 male NPTF threads on one end and 1/4-18 female NPTF threads on the other.

trying to swap out a street 45, is this one male on one side and female on the other, both 1/4-18 NPTF?

For fitment, it is a 45 degree street elbow with 1/4-18 Male NPTF on one end and 1/4-18 Female NPTF on the other, built to SAE J476 thread specs.

both ends are 1/4-18 NPTF on this thing, so can i thread the male end straight into a female port and then run another fitting off the female side?

That's how a street elbow works. The male NPTF end threads directly into a female port or fitting, and then the female NPTF end on the other side accepts the next male fitting in your line. Both threads are 1/4-18 and meet SAE J476 spec, so as long as your mating fittings are the same standard, you've got a sealed, leak-resistant connection on both ends without needing a coupler or adapter in between.
