got a tight spot on a brass run, what exactly is this fitting and what threads are on it?

For tight hydraulic connections layouts, it gives you a 45 degree change in direction using a street elbow layout, meaning one end is male and the other is female. Both ends are NPTF threads to SAE J476, sized 1/4-18.

both ends on this thing are 1/4-18 NPTF right, or is one end a different size?

Both ends are the same size, 1/4-18, and both are NPTF thread type. One end is male and the other is female, which is what makes it a street elbow rather than a standard elbow. That male-to-female configuration lets you thread it directly into a fitting or port without needing a separate coupling in between.
