trying to swap out an elbow on a big line, are both ends on this one 1-1/2 male nptf?

Both ports are 1-1/2-11-1/2 male NPTF threads, and they follow SAE J476. It is a 90 degree elbow with male NPTF on each end.

both ends on this thing are 1-1/2 inch male NPTF right? just want to make sure before I order

Correct on both ports. Each end carries 1-1/2–11-1/2 male NPTF threads built to SAE J476, so you're getting a matching dry-seal fit on either side. No mismatched sizes to worry about with this one.
