trying to hook up a tee in a tight spot, what thread is on all three ports of this run tee?

All three connections are M20 x 1.5 DIN tube threads. Two sides are male (S12) and the third side is a female (S12) swivel.

All three ports on this thing run M20 x 1.5 threading? Just want to make sure before I go ordering a bunch of fittings to match up with it.

All three connections on the 5390S-12 use M20 x 1.5 thread, so you only need one thread spec across the board when sourcing your mating fittings. Two of the ports are male DIN tube ends and the third is a female DIN tube port with a swivel design, which gives you some wiggle room during installation for getting alignment right without fighting the whole assembly.
