What exactly is the thread setup on this fitting, both sides M16 x 1.5, or are they different?

Both sides run M16 x 1.5, but they're not the same end. Side 1 is a male DIN tube connection and Side 2 is female, both at L10 size. The 45-degree angle between them is what makes it useful when you need to redirect a line without putting a straight adapter in a tight spot.

trying to swap out a bent elbow on a DIN tube line. is this one male M16 x 1.5 on one end and a female swivel M16 x 1.5 on the other?

It matches a DIN tube connection with an M16 x 1.5 male on Side 1 and an M16 x 1.5 female swivel on Side 2, both in size L10.
