fitting says 4S series and NPTF male end, does that half-inch size refer to the thread connection or the hose end?

The 0.5-inch measurement is the NPTF male end connection size. The hose side carries a dash-8 designation, which is the standard way hose fittings reference the hose end sizing in that series. So you're looking at two different sizing references on the two ends, which is pretty common with crimp fittings like this one.
