fitting says 22.5 degree angle, does that mean it ain't gonna sit flush like a standard straight flange would?

That angle is intentional. The 22.5 degree offset lets you route hose or tube in tighter spaces where a straight flange would put too much stress on the line or just plain wouldn't clear surrounding components. It's a split flange design, so it clamps around the flange head rather than threading on, which keeps everything secure even with that angular offset. If your current setup has a straight Code 61 flange port, this fitting will still mate to it correctly. The angle affects the direction the hose exits, not the port connection itself.
