trying to swap out an elbow on a line, is this one the 7/8-14 UNF JIC male on one end and M18 x 1.5 male metric on the other?

On the JIC side, you get a 7/8 - 14 UNF male connection (5/8 inch size). The other end is a male metric M18 x 1.5, listed as an 18 mm size, and the fitting is a 45 degree adjustable elbow.

what's the thread spec on the metric side of this fitting, and is it a standard pitch or something unusual?

M18 x 1.5 is the callout on the metric end, which is a fine pitch and fairly common in European-style hydraulic equipment. The male metric side is adjustable, so you can clock it to the angle you need once it's in place. The JIC side runs 7/8-14 UNF, which is a standard 37-degree flare connection at 5/8" size.
