Does this fitting go from a JIC flare on one side to a pipe thread on the other, or am I reading that wrong?

You're reading it right. One end is a #4 Male JIC, which uses a 37-degree flare seat, and the other end is a #4 Female Pipe thread. That combination lets you connect a JIC-style hydraulic line directly into a pipe-threaded port without needing a separate adapter in between.
