trying to swap out an old fitting, what are the two ends on this Aeroquip 2021 and what thread sizes are they

For fit-up, it is a straight steel adapter with a male JIC SAE 37 degree flare on one end and a male NPTF on the other. The JIC side is 1-3/16-12 (SAE J514) and the NPTF side is 3/4-14 (SAE J476).

fitting's rated for 5,000 PSI dynamic pressure -- will that hold up running hydraulics on a skid steer or loader all day long?

At 5,000 PSI maximum dynamic pressure, this fitting is built for exactly that kind of continuous working load. Heavy equipment hydraulics on skid steers and loaders typically operate well within that range, so you're not pushing the limits here. Straight steel construction holds up under the heat cycling and vibration that comes with that kind of duty. Just make sure your line connections on both ends match up -- 1-3/16-12 JIC flare on one side and 3/4-14 NPTF on the other.
