thread sizes on this one, is it a 3/4-16 on one end and 3/8-18 on the other, or am I reading that wrong?

You've got it right. One end is a 3/4-16 Male JIC with a 37-degree flare, conforming to SAE J514. The other end is a 3/8-18 Male NPTF, built to SAE J476. So yes, two different thread sizes, which is exactly what this straight adapter is meant to handle when you're stepping between those two connection types.
