Does this adapter handle the size difference between 1/8 inch pipe thread and a 1/2 inch JIC fitting, or do I need something in between?

That's exactly what it bridges. One end is 1/8 inch NPTF male thread and the other is 1/2 inch SAE 37 degree male JIC, so it connects those two sizes directly with no additional fittings required. The 45 degree elbow angle also helps when you're working in tight quarters and need to route a line without putting a hard bend on the hose.

trying to replace an old elbow adapter, what are the thread and flare ends on this one?

On one side you have a 1/8 inch NPTF male pipe end, and on the other side it is a 1/2 inch SAE 37 degree male flare (MJIC). It is a 45 degree elbow adapter.
