this is a Monarch Industries cylinder, but does the 2 x 16 mean a 2-inch bore with a 16-inch stroke, or is that measured some other way?

Correct on both counts. The 2 refers to a 2-inch bore diameter and the 16 refers to a 16-inch stroke length. That's standard tie rod cylinder sizing notation across most manufacturers, and Monarch follows the same convention.
